It is with a heavy heart that we share our clinic will be closing on August 21, 2026. We are truly sad to be closing and will miss our patients and families more than words can express.

If you would like a copy of your child’s most recent evaluation to take with you, please complete the Medical Release form linked below.

Please note that a signed Medical Release form is required before we are able to release or send any evaluation records.
